Current Law

Persons are eligible to receive their original birth certificate under current law if they meet the following criteria:

  • Born and adopted in Connecticut
  • Is at least 18 years old

Pursuant to the law, persons are also entitled to receive a Contact Preference Form and Medical History Form, if they have been filed by his/her biological parents with the Department of Children and Families.

Persons must file an application and pay a $65 fee to the town clerk of the town where they were born to receive their original birth certificate.  This is the application form. Here is a list of all the town clerks and their addresses.

Adoptees who do not know what town they were born in may contact the Department of Children and Families (DCF) at (860) 550-6319. DCF maintains an index of all adoption placements since 1915 and will be able to provide this information.

Descendants of adoptees who are at least 18 years old are also entitled to request the original birth certificate of their adopted parent/grandparent.
